I know that the PCSX2 core is beta, but this is something I've never discovered with any other PS2 game on Retroarch. Turning off cheats in the Quickmenu Options, or renaming the PNACH file slightly (so it's not the exact CRC of the game) lets the game run normally, with the video present, but even if RetroArch recognizes there is a matching PNACH file with the correct CRC, there's no video just a blank white screen in the emulator, despite all the codes being set to 'patch=0' (disabling them). Not that there are any really useful cheat code for the PS2 game, 'ICO', but in trying to just get a PNACH file to work with the game, I found that any time I try to run the game with a cheat file present causes me to get nothing but a blank white screen in the emulator, even as the audio plays properly.
